I, declare on my responsibility that this is a teaching activity free from any commercial ties and will not include any brand names. I will inform the Order of Nurses of any change in the program or change of speakers. I will send within 2 weeks of completing the activity , the list of nurses who attended including the following:

Nurse full name
ONL ID number
Contact number and email
Place of work
Time of log in and log out of each attendee if online activity
